Information you provide
We process your contact details, inquiries, signed consents and intake answers to respond to you and arrange care. Please use the secure patient portal for clinical messages. Do not use website forms for emergencies.
Forms and email
We store completed consent forms, intake answers and related requests in encrypted storage with restricted access. Completion records help returning patients avoid repeating the current patient agreement and intake. You can download your signed copies through your private link. Email contains notifications and private links, without completed-form attachments. Anyone with your link may be able to open your forms until it expires or is revoked, so protect your email account and links. Clinical messages and follow-up use PushHealth. Email providers may retain notification messages under their policies.
Technical information
The website uses an essential, temporary cookie to keep your private form session open. Our hosting and security systems may process connection details to operate the service and prevent abuse. We do not put your form answers in advertising analytics or browser draft storage.
Sharing and choices
We share information with service providers involved in hosting, email delivery and clinical services, and as required or permitted by law. We do not sell personal information. Ordinary email may not be encrypted and can be read by someone with access to your account. You can ask the clinic for a different signing or delivery method.
