The basics

Tirzepatide works on two of your own gut hormones at once, GLP-1 and GIP. By stretching out the signals your intestine sends after a meal, it quiets appetite, slows stomach emptying, and steadies blood sugar.

In SURMOUNT-1, a 72-week trial in adults with obesity, average weight loss ran from about 15 to 21 percent of body weight across the dose range. Those are full-dose numbers, and in practice the best dose is the lowest one that gets you results you can live with, which is why we micro-dose and titrate slowly.

We review the studies, the potential uses and benefits, and the open questions in our research post, Micro-Dose Tirzepatide and L-Carnitine for Steadier Weight Loss.

What the evidence looks like

Strong. Tirzepatide has large randomized trials for weight loss and type 2 diabetes. SURMOUNT-4 showed that people who stopped regained much of the weight, so we pair the medication with labs, follow-up, and habit change.

Why patients ask about it

  • Weight loss with fewer side effects through micro-dosing
  • Insulin resistance and blood sugar control
  • Protecting muscle and metabolism while losing fat

Good to know

  • FDA-approved as Mounjaro and Zepbound. The compounded tirzepatide with B12 we prescribe is not FDA-approved.
  • Common side effects are nausea, constipation, and reflux, usually during dose increases.
  • Not for anyone with a personal or family history of medullary thyroid carcinoma or MEN2, or during pregnancy.
